Discretionary & Hauora Grant Application Form

Purpose

The purpose of the Ngāti Mākino Hauora/Discretionary Grant is to;

    • Improve the health and wellbeing of Ngāti Mākino

Allocation

    • Individual grants will be paid at the trustees discretion

Eligibility & Criteria

    • Applicants must be registered on the Ngāti Mākino Iwi Register or are a direct descendant of someone who is registered on the Ngāti Mākino Iwi Register
    • A Ngāti Mākino hauora or discretionary grant can be applied for at any time
    • Grants are available for
        • contribution towards the cost of prescription glasses, dentures, smoking cessation and hearing aids
        • contribution towards the repair of glasses, dentures and hearing aids
        • to support representation at provincial or national levels e.g. sporting or leisure pursuits
    • The grant can then be paid directly to the health provider or an applicant can be reimbursed upon providing a receipt from a health provider
    • Only one grant is payable to an applicant in a 12 month period
    • The NMHT may decline any application on any grounds it sees fit
    • Approval of any grant is entirely at the discretion of the Trustees

Applicants Must

    • Provide a whakapapa if required
    • Complete a hauora/discretionary grant application form, available from the Ngāti Mākino website or Ngāti Mākino office
    • For hauora grants an applicant must attach a quote or receipt with their application
    • For discretionary grants, supporting information must be provided
    • Write an essay about their future goals including how they will help Ngāti Mākino
    • Be willing and make time available to attend Ngāti Mākino wananga/hui
